Hallo,
ich habe mal versucht, das Beispiel im MailformPlus (example_form/captcha) bei mir einzubinden.
Das Resultat zeigt mir jedoch kein Bild an. Hier mal der Quelltext vom Resultat:

So wie es aussieht, gibt es wohl ein img-Tag mit Breite und Höhe von 0.
Rufe ich den Link typo3conf/ext/captcha/captcha/captcha.php direkt auf, dann wird ein Bild generiert.
Ich habe das Beispiel 1:1 übernommen und auch TS so übernommen:
