realUrl fehler

Hallo Forum
leider funktioniert mir trotz manual, google und Foren wie #66633 mein realURL immer noch nicht richtig. Es zeigt mir immer noch www.domain.ch/typo3/.html

Ich habe sehr viel ausporbiert und vermute ob was grundlegendes fehlt. Wie die aktivierung im appache httpd.conf. Reicht es nicht dies per /typo3/.httaccess zu lösen oder muss ich wirklich was in httpd.conf schreiben?

Habe bisher folgendes ausprobiert:

- Typo3 4.1.5, realUrl 1.5.1
- Server extern
- Enable automatic configuration aktiviert
- simulateStaticDocuments = 1 getestet:
Ergebnis: domain.ch/typo3/kontakt oder
domain.ch/typo3/12.html oder
domain.ch/typo3/12.0.html
Ist damit nicht auch bewiesen dass modRewrite aktiv ist?

Meine jetzige Konfig:

.htacces auf /typo3

(habe auch schon einige andere (&minimale) konfigs ausprobiert)

Setup Root seite:

simulateStaticDocuments = 0
baseURL = http://kasche.ch/typo3/
tx_realurl_enable = 1
prefixLocalAnchors = all

und als letztes eine noch den folgenden Eintrag in /realurl/ext_localconf.php.
Meine Root-Seite hat die ID=1
