Flexform in Extbase Plugins

Hallo Community,

ich würde gerne für meine "Extbase/Fluid Extensions" ein Plugin Formular, welchen ich über Flexforms einbinden wollte, erstellen.
Aber leider wird im Plugin keine Flexform angezeigt. Cache geleert und die Schreibrechte stehen auf 0755.
Ich verwende Typo3 4.5.27 (Extbase 1.3.4)
Hier die exttables.php:
